Navigating health benefits can be tricky, so that’s why our team of certified navigators can work with you to make sure you get the health coverage you need. Our health benefits specialists are available to talk to you anytime throughout the year. We’ll explain what plans you qualify for and help you get enrolled.

We’ll help you get to your appointment

If you’re having trouble accessing transportation, we can help. When you call to make an appointment, ask our phone operator about how we can reimburse you for your travel cost. Also, your insurance might provide transportation assistance benefits for non-emergency medical visits.

Easy access to the Family Case Management (FCM) and Women, Infants and Children Program (WIC)

The Family Case Management program can help connect expectant mothers or families with a baby under the age of 1 to essential services such as medical services, childcare, transportation, housing, food, and mental health needs and/or substance use disorder services. The program also offers a free baby shower to enrolled expectant mothers.

The WIC program is a federal nutrition program offered through the U.S. Department of Agriculture (USDA) and administered in Illinois by the Department of Human Services (DHS). If you are pregnant, breastfeeding, have an infant or a child under five, and are working with a limited income or have no income and need help providing food for your family, enrolling in these programs can make a big difference in your life.

What’s great about the FCM and WIC programs is that it helps parents to monitor and feed their children properly during critical periods of growth and development. These programs provide education and support, and access to nutritious foods and other essential services.
