Fees & Payment

Our fees are based on the procedures done at the time of service. All fees are determined by the procedures rendered by the provider, with the standard fee reflecting the current medical marketplace. We accept many different forms of payment, including self-pay, insurance, Medicaid Managed Care plans, Medicare, and most HMOs and PPOs. Payment is expected at the time of service, cash, checks, and major credit cards are accepted. You may find ways of paying for your visit below.

At Greater Family Health, no one will be turned away for an inability to pay.

Self-Pay

Greater Family Health makes accessing care easy. If you do not have insurance or are underinsured, you may qualify for the Sliding Fee Discount program. As a Community Health Center grantee, we offer a sliding fee discount program to all patients who are under 200% of the Federal Poverty guidelines based on reported income and family size.

All patients presenting who are below 100% of the Federal Poverty guidelines based on reported income and family size will receive a full discount. However, nominal fees may be collected from individuals or families with an annual income at or below 100% of the Poverty Level. Every patient below 100% of the Federal Poverty guidelines pays only a Nominal Fee each visit – which includes laboratory testing services.

To qualify for the sliding fee discount, patients must provide proof of identification, proof of family size, and proof of income. Patients must renew this information every 12 months to continue to receive the discount.
