The Use of Case Management in Providing Continuity of Care

Greater Family Health is accredited by The Joint Commission, indicating compliance with the highest quality standards of health care in the United States. Greater Family Health is a certified Primary Care Medical Home (PCMH) and provides care regardless of our patient’s ability to pay.

Our services are provided by Primary Care Providers (PCP) who are responsible for the provision of ongoing health care. Continuity of care is assured by our Case Managers, who coordinate care across all health care settings including specialty care, hospital care, urgent or emergency care and community agency assistance.
Our Case Managers are therefore a critical element of the Care Team, coordinating services to achieve the best outcomes for our patients through the utilization of care coordination, internal and external resources and a team-based approach.
Case Management Services Include:

We have the privilege of providing care for over 36,000 people annually. Data released by the Bureau of Primary Health Care indicates that Greater Family Health’s outcomes continue to be far better than most Community Health Centers on a wide range of health care outcomes, financial outcomes and operational items. The achievement of these positive outcomes is partly due to our successful coordination of patient care.
