Activate Elgin is an initiative to engage all sectors of the community to work in harmony providing all residents with opportunities to improve their health, well-being, and overall quality of life. Greater Family Health proudly serves on the Activate Elgin Coalition.

AMITA and Greater Family Health enjoy a rich and substantial collaboration. Greater Family Health leases space from AMITA for our location in Hanover Park. In addition, our Obstetricians perform newborn deliveries and surgeries at both Ascension St. Alexius Medical Center and Ascension Alexian Brothers Medical Center.

Ascension St. Joseph Hospital has been collaborating with Greater Family Health since before we were even incorporated, helping drive community support to incorporate the budding organizations and encourage its application to become a Federally Qualified Health Center.

Greater Family Health provides its quality, affordable primary health care services to homeless persons from Elgin with financial support from Aunt Martha’s.

Greater Family Health offers both primary care and dental care to Boys & Girls Club members and parents through their annual health fairs.

Greater Family Health and CEDA’s WIC (Women, Infant and Children) program often refer clients/patients to each other for services.

Greater Family Health provides health education for Centro clients at Centro’s offices twice monthly. Additionally, we offer both primary care and dental care to Centro clients through their annual health fairs.

Greater Family Health and Children’s Home + Aid often refer clients/patients to each other for services.

The professionals at our McHenry location are actively working on a committee formed by 4-C to improve access to services available to those children of McHenry County in need.

CCHHS offers diagnostic and subspecialty services to Greater Family Health patients who reside in Cook County, including those that receive care at our locations in Streamwood and Wheeling.

DeKalb County Health Department was instrumental in the planning and development of our DeKalb location, which provides pediatric and family medicine care in the Health Department’s offices at 2550 N. Annie Glidden Road in DeKalb.

DeKalb and Sycamore area patients who need preventive and restorative oral health care are referred to DeKamore Dental for services, in addition to our own Dental Clinic.

 

Greater Family Health provides school-based oral health care in a number of area schools each year.

Ecker Center for Behavioral Health is where Greater Family Health refers many Elgin-area adult patients who are in need of outpatient behavioral health services. Conversely, Ecker Center refers their clients to Greater Family Health for primary health care, including adult oral health care, as needed.

Wheeling-area patients who need preventive and restorative oral health care are referred to familia Dental for services, in addition to our own Dental Clinic.

In addition to Greater Family Health referring behavioral health patients to FSA for ongoing counseling and FSA referring clients who need primary health care to us, the two organizations enjoy a rich collaborative history regarding advocacy for underprivileged persons.

Greater Family Health regularly provides informational services at Food for Greater Elgin for area residents visiting their location. Conversely, Food for Greater Elgin educates their guests about the quality and affordable primary health care services available at all our Health Centers.

Greater Family Health provides a full range of ambulatory health care services for victims of domestic violence and abuse, including their children, who are residing at the shelter on a regularly scheduled basis. Community Crisis Center – General P: (847) 742-4088

Greater Family Health serves alongside the Kane County Health Department to address the health needs of the residents of Kane County through the Health Department’s Advisory Board, Healthy Places Coalition, All Our Kids (AOK) Network, the Kane County Family Case Management Cluster and the Kane County Breastfeeding Coalition, to name a few.

Greater Family Health provides primary health care, oral health care and behavioral health services, including substance use disorder services, to participants of the Pre-Arrest Diversion Program offered by the County. 

LabCorp leases space in our Health Centers, providing a comprehensive Patient Service Center for Greater Family Health patients needing any number of laboratory services via a bi-directional laboratory interface.

MEDPRO provides mobile ultrasound staffing and services in several of our Health Centers.

Greater Family Health partners with the Northern Illinois Food Bank to host mobile food banks in Elgin, IL and Sycamore, IL.

Greater Family Health has had a long-standing relationship with Northwestern Medicine and its Kishwaukee, McHenry and Huntley Hospitals.  This includes our provision of deliveries/surgeries at Northwestern Medicine Huntley Hospital.  Northwestern Medicine offers a wide array of specialty and ancillary services to our DeKalb, DuPage, Kane and  McHenry county patients. The two organizations regularly refer patients to each other for services the other provides.

NCH was instrumental in the planning and development of our location in Wheeling. NCH offers a wide array of diagnostic and subspecialty services to Wheeling area patients, who also deliver their newborns at NCH. The two organizations regularly refer patients to each other for services the other provides.

As a premier provider of mental health and substance use disorder services for area residents, younger patients from our Wheeling area are referred to Omni for care, as needed.

Professionals from our McHenry location collaborate with the Youth Service Bureau of Pioneer Center to help provide management and continuity of care for some of the children and adolescents that the Bureau manages. In addition, Greater Family Health and Pioneer Center often refer clients/patients to each other for services.

Greater Family Health and Rosecrance refer clients to each other and collaborate regarding health education and service provision.

Greater Family Health serves on Safe from the Start Coalition and provides referrals to the Safe from the Start program.

Greater Family Health and Thresholds extensively refer clients to each other and collaborate regarding health education and service provision.

Greater Family Health offers both primary care and dental care to Two Rivers Head Start Agency students and parents through annual health fairs.

Greater Family Health provides school-based oral health care in a number of area schools each year. Representatives from Greater Family Health also serve on U-46’s Civic and Business Advisory Committee.

VNA jointly participates with Greater Family Health in the Kane County Family Case Management (FCM) Cluster, and the two organizations also work together regarding breast and cervical cancer screening services and referrals to Greater Family Health for oral health care services at our Dental Clinic.

Through their participation in the Federal Drug Pricing (340B) program, Walgreens offers Greater Family Health patients reduced-priced pharmaceuticals at all of their locations in our service area.
