We have always offered a sliding fee discount program to all patients who are at or below 200% of the Federal Poverty guidelines based on reported income and family size. In order to qualify for our discount program, patients are required to provide their proof of income.
PROOF OF INCOME EXAMPLES
Paycheck stubs reflecting 30 days, W2, Income Tax Return, Social Security Benefit letter, Unemployment Letter or Statement, Letter of Support if there is no income, Letter from Employer if paid in cash and Benefit letter from Link Card are all example of acceptable proof of income.

HOW MUCH WILL IT COST FOR MY VISIT?…Fees are based on office services completed at the time of service, which often include diagnostic laboratory testing. Although testing is key to improving healthcare quality and guides more than 70 percent of medical decisions for personalized care, many of our patients just can’t afford laboratory diagnostic testing.

Thorough efforts with our laboratory partners, we are now able to reduce the financial burden of your laboratory testing. Every patient below 100% of the Federal Poverty guidelines now pays ONLY a Nominal Fee of $25.00 each visit – which includes all laboratory testing services – over 37% less than any other FQHC provider.
