Greater Family Health Receives Multiple National Awards for Quality of Care – Again!

Greater Family Health’s 2020 outcomes on Bureau of Primary Health Care Clinical Indicators have been recognized as among the best of all Federally Qualified Health Centers in the United States and Illinois – for the 8th year in a row!

 

The Department of Health and Human Services (HHS), Health Resources and Services Administration (HRSA) recognized Greater Family Health with 6 National Awards on August 30, 2021:

  1. Health Center Quality Leader – Silver
  2. Access Enhancer
  3. Health Disparities Reducer
  4. Advancing Health Information Technology for Quality
  5. Patient Centered Medical Home
  6. COVID-19 Reporter

Greater Family Health’s clinical outcomes ranked in the top 15% of the Country’s 1,375 reporting Federally Qualified Health Centers, resulting in the receipt of the Health Center Quality Leader – Silver Award.  Greater Family Health has received the National Health Center Quality Leader Award for 8 consecutive years, and the Advancing HIT for Quality Award every year it has been offered. 

Greater Family Health also remains a 2017 Million Hearts Hypertension Control Champion, a 5-year honor, awarded by the Centers for Disease Control and Prevention.

 

Coupled with operational and financial comparison reports issued on August 10, 2021, the Awards acknowledge that Greater Family Health’s outcomes are far better than other Health Centers’, both in the State and Nationally, on an array of clinical and financial indicator outcomes.

The released comparison data indicate that Greater Family Health also has the 2nd lowest cost per patient of the 45 FQHCs in Illinois.  Further:

  • Greater Family Health’s Total Cost per Total patient is 47% less than the State average and 142% less than the National average
  • Greater Family Health’s Medical Cost per Medical Patient is 35% less than the State average and 48% less than the National Average
  • Greater Family Health’s Dental Cost per Dental Patient is 29% less than the State average and 41% less than the National Average
