Greater Family Health Nationally Recognized as Quality Leader

Elgin, IL – Greater Family Health was recognized by U.S. Department of Health and Human Services (HHS) Secretary Sylvia Burwell today as a Health Center Quality Leader.

Greater Family Health has therefore been awarded additional federal funding for being both a Clinical Quality Improver and a Health Center Quality Leader. Greater Family Health is 1 of only 14 Community Health Centers in Illinois to be recognized as a Quality Leader, receiving the 5th highest awarded amount in Illinois, amongst 42 eligible organizations.

Citing Greater Family Health’s proven track record of clinical quality improvement and provision of the high quality primary care services, Greater Family Health was specifically recognized for demonstrating at least 10% improvement in at least 1 clinical quality measure year-over-year, for being among the top 30 percent of all Health Centers for achievement of the best overall clinical outcomes, and for demonstrating the ability to focus on quality in all aspects of clinical operations.

Using 2013 data, HHS cited Greater Family Health as a proven leader in areas such as chronic disease management, preventive care and use of Electronic Health Records to report quality data.

Greater Family Health is accredited by The Joint Commission, indicating compliance with the highest quality standards of health care in the United States, certified as a Patient Centered/Primary Care Medical Home and provides care regardless of the patient’s ability to pay. The organization offers care via 7 full-time Health Centers and an additional 22 part-time locations through the efforts of over 50 Licensed Independent Practitioners and over 150 supportive personnel. In Calendar Year 2014, Greater Family Health will provide care to about 38,000 different persons through over 120,000 appointments.
