For the fourth year in a row and for the period ending March 31, 2013, Greater Family Health exceeded Illinois Health Connect Quality of Care benchmarks. As a result, Greater Family Health was recognized by the Illinois Department of Healthcare and Family Services for the high quality of care it provides patients.
Greater Family Health practitioners provided health care services that resulted in patient outcomes exceeding quality benchmarks on 8 different outcomes:
- Percent of children under age 2 who received all their immunizations
- Percent of children under age 1 receiving development screening
- Percent of children under age 2 receiving development screening
- Percent of children under age 3 receiving development screening
- Percent of children under age 2 receiving lead screening
- Percent of children and adults receiving proper asthma treatment and medication use
- Percent of women between 42 – 69 who have received breast cancer screening
- Percent of patients 18 – 75 who receive diabetic HbA1C testing
These outcomes and the State’s recognition further documents that Greater Family Health provides the highest quality of patient care!
