2014 data released by the Bureau of Primary Health Care indicates that Greater Family Health’s outcomes continue to be far better than most community health centers (CHC) on a wide range of health care outcomes, financial outcomes and operational indicators.
The UDS Summary Report, released earlier this week by the Health Resources and Services Administration, articulate the clinical and financial performance outcomes of Greater Family Health, comparing them to State of Illinois and National community health center benchmarks.
For the 10th consecutive year, Greater Family Health out-performs its peers:

Greater Family Health’s Health Care Outcomes beat the Illinois average on 11 of 15 outcomes:
- Access to Prenatal Care
- Low Birth Weight
- Child and Adolescent Weight Assessment and Counseling
- Tobacco Use Screening and Cessation Intervention
- Colorectal Cancer Screening
- Depression Screening and Follow-Up
- Asthma Treatment
- Cholesterol Treatment
- Heart Attack/Stroke Treatment
- HIV Linkage to Care
- Blood Pressure Control
Financial Indicators:
Greater Family Health’s Financial Outcomes also beat the Illinois average on every financial indicator, meaning that Greater Family Health provides quality health care at a low cost:
- Greater Family Health’s medical cost per medical patient is 19% lower than the State CHC Average and 27% lower than the National CHC average
- Greater Family Health’s medical cost per medical visit is 28% lower than the State CHC average and 31% lower than the National CHC Average
- Greater Family Health’s total cost per total patient is 29% less than the State average and 43% less than the National CHC Average
- Greater Family Health’s total cost per total health care visit is 26% less than the State average and 32% less than the National CHC Average
- Greater Family Health’s dental cost per dental care visit is 28% less than the State average and 39% less than the National average
Operational Indicators:
The 2013 Report indicates that the volume of care provided by Greater Family Health practitioners is rapidly growing:
- Growth in total number of patients cared for over the past 3 years is 8 times the State of Illinois average and 6 times the National average
- Growth in total number of dental patients served over the past 3 years is 6 times the State of Illinois average

- Greater Family Health is a non-profit Community Health Center which provided care to over 37,500 different people via almost 126,000 appointments in 2014.
