Greater Family Health is scheduled to host an enrollment event to help area residents learn more about the new health insurance options available through the Affordable Care Act and enroll into a health insurance plan.
Where: Boys and Girls Club of Elgin, 355 Dundee Ave, Elgin, IL.
- Proof of residence ( Driver’s license, State ID, etc)
- Proof of income ( W2, pay stubs)
- Health insurance policy numbers (if currently insured)
- Social Security/Resident ID number
- Date of Birth
Greater Family Health is dedicated to spreading the word about Get Covered Illinois and the range of health insurance options and community resources that are available to area residents through the Affordable Care Act.
Specially trained counselors, called Navigators, will be on-site to assist and answer questions.
