Greater Family Health’s 2017 outcomes on the Bureau of Primary Health Care’s Clinical Indicators have again been recognized as among the best of all Federally Qualified Health Centers in Illinois and the United States – for the 5th year in a row!
The Department of Health and Human Services, Health Resources and Services Administration (HRSA) recognized Greater Family Health as a Health Center Quality Leader on August 22, 2018, with Greater Family Health being awarded its 5th consecutive Health Center Quality Leader award.
Greater Family Health also received the prestigious Advancing Health Information Technology for Quality award, in recognition of Greater Family Health’s comprehensive use of integrated HIT, both internally and through interoperable IT communication systems with external collaborating organizations. Also awarded to Greater Family Health was the Primary Care Medical Home Recognition and Electronic Health Record Reporter awards.
The Awards are based on Greater Family Health’s internal sophistication and 2017 clinical outcomes, which substantially outpaced the Illinois benchmark on 13 of 16 reportable Clinical Indicators. In addition, Greater Family Health’s clinical outcomes were also better than the National average of 13 of 16 Indicators.
Greater Family Health was also recognized by the Centers for Disease Control as a 2017 Million Hearts Hypertension Control Champion.
The recognitions again acknowledge that Greater Family Health’s operations and outcomes are superior to those of most Health Centers in the State and Nationally.
