Diabetic Care

Diabetes is a chronic condition where the body has difficulty regulating glucose, commonly known as blood sugar.

Normally, insulin, a hormone produced by the pancreas, helps break down the glucose from the foods you eat into energy. In patients with diabetes, either the body doesn’t make enough insulin (known as type 1 diabetes) or the body’s cells don’t respond properly to insulin (known as type 2 diabetes).

High blood sugar levels, known as hyperglycemia, can cause damage to blood vessels and nerves over time. This can lead to or exacerbate various health problems like heart disease, vision loss, and kidney disease.

While diabetes cannot be cured yet, it can be managed with healthy lifestyle choices, blood sugar management, and regular monitoring. This can be achieved by eating a balanced diet, regular exercise, medication adherence, and regular checkups.

Our dedicated staff at Greater Family Health play a vital role in supporting healthier outcomes for our patients impacted by diabetes. Our team of providers work one-on-one with their patients to manage medications and offer the resources needed for long-term success.

One component of this is the Diabetic Self-Management Goals. These goals are completed annually by the provider and patient working together to create realistic goals that are manageable for each individual patient and their lifestyle. Greater Family Health offers a comprehensive Diabetic Education Folder (in both English and Spanish) as well as a blood sugar tracking log to monitor your blood sugars at home. We also provide point of care testing that allows us to check your A1c level with a quick fingerstick and results in just a few minutes!
