Transforming Access into Impact: A Success Story

At Greater Family Health, improving access to care is what we do. John’s story is a powerful example of how integrated, patient centered care can make a major difference.


During a routine visit with one of our Family Practice Providers in Wheeling, John completed a Patient Health Questionnaire-9 (PHQ-9), a common screening tool for depression and anxiety. His results indicated elevated concerns, prompting immediate attention from his care team.


In the past, patients like John might’ve faced delays due to referrals and additional screening steps. But thanks to recent improvements in our care model, those barriers have been removed. It’s now much easier for patients to access behavioral health services.


That same day, John was able to meet with a Licensed Clinical Social Worker on-site.From that moment forward, his journey began to shift.

Through ongoing therapy sessions, John developed effective tools to manage the anxiety he lived with everyday. He was even connectd with psychiatric mental health services and began a treatment plan that included medication to support his progress.


Over time, John has experienced steady improvements in both his emotional and physical well-being. He’s learned to manage distress, reduce anxiety symptoms, and build long-term coping strategies that help him in his day to day life. Even his routine medical visits look different. When faced with moments that normally spiked his anxiety (like getting his vitals taken) John now applies breathing techniques he learned in therapy. John’s experience shows the importance of timely, integrated care and the powerful outcomes it can produce.
