Primary Care Medical Home, or PCMH for short, is a designation that Greater Family Health worked very hard to achieve.
Greater Family Health became certified as a Primary Care Medical Home in 2007 by The Joint Commission, an independent, not-for-profit organization whose accreditations and certifications are recognized nationally as a symbol of quality and high medical standards. To be a PCMH means that our patients are at the center of what we do.
