Care & Case Management Programs
We understand the complexities of navigating the health care environment. Greater Family Health offers several care/case management programs which effectively help our clients and patients receive additional services and care according to their individualized treatment and services plan.
Full-Service Laboratory
If your doctor orders additional testing, you can access the on-site laboratory services provided by LabCorp.
LabCorp serves all Greater Family Health locations.
Discounted Pharmaceuticals
Talk to your provider at your next visit to see if you qualify.
Transportation Assistance
Be sure to check with your insurance, as you may also have transportation assistance benefits for non-emergency medical visits through your insurance plan.
Electronic Medical Records
At Greater Family Health, we believe in treating our patients’ entire range of healthcare needs, from medical and dental to behavioral health.
We use Electronic Medical Records to facilitate coordination and efficient care for our patients. Additionally, our Patient Portal helps improve patient and provider interaction and communication.
Women, Infants and Children Program (WIC)
WIC is a federal nutrition program offered through the U.S. Department of Agriculture (USDA) and administered in Illinois by the Department of Human Services (DHS). WIC helps parents to feed their children properly during critical periods of growth and development. This program provides nutrition education and counseling, breastfeeding support, and access to nutritious foods.
Ask your provider about this program if you are pregnant, breastfeeding, have an infant or a child under 5, and are working with a limited income or have no income and need help providing food for your family.
