Funding Received For Health Center in Streamwood

On June 20, 2012, Greater Family Health was informed that it had received a New Access Point grant from the Bureau of Primary Heatlh Care to support the opening and operation of a Health Center in Streamwood, Illinois.

Funds from the grant will be used to complete the construction of the Health Center, located at 135 E. Irving Park Road in Streamwood, as well as to support the hiring of 3 full-time medical practitioners and several support professionals.

The Health Center will open at 8:00 am on Wednesday, August 8, 2012.

The Health Center will offer Pediatric care, Family Practice care and Prenatal Care, as well as reduced-price pharmaceuticals, a full-service laboratory, case management and several other support services.
The Health Center is conveniently located to those residing in Hanover Park, Streamwood, Bartlett, South Elgin, Hoffman Estates, Schaumburg, Bloomingdale and Carol Stream. All care will provided regardless of the patient’s ability to pay.
