As of November 1st, 2017, Family Health Network (FHN) will not be participating in the Illinois Medicaid Managed Care Program. If you are insured with FHN and reside in Cook County, you will be transitioned to CountyCare Health Plan.
If you wish to stay with CountyCare Health Plan, you do not need to do anything. If you do nothing, you will be insured by CountyCare Health Plan and be able to continue to use Greater Family Health’s Health Centers as your Medical Home/Primary Care Physician.
If you DO NOT want to stay with CountyCare, you have 90 days from November 1st, 2017 to change health plans to any of the plans listed below.
Greater Family Health accepts all the plans below. Blue Cross Community Health Plan is our preferred plan.
