Greater Family Health’s 2020 outcomes on Bureau of Primary Health Care Clinical Indicators have been recognized as among the best of all Federally Qualified Health Centers in the United States and Illinois – for the 8th year in a row!

The Department of Health and Human Services (HHS), Health Resources and Services Administration (HRSA) recognized Greater Family Health with 6 National Awards on August 30, 2021:

Greater Family Health’s clinical outcomes ranked in the top 15% of the Country’s 1,375 reporting Federally Qualified Health Centers, resulting in the receipt of the Health Center Quality Leader – Silver Award.  Greater Family Health has received the National Health Center Quality Leader Award for 8 consecutive years, and the Advancing HIT for Quality Award every year it has been offered. 

Greater Family Health is also a 2017 Million Hearts Hypertension Control Champion, a 5-year honor, awarded by the Centers for Disease Control and Prevention.

Operating 9 Community Health Centers in 5 northeastern Illinois counties serving residents of 520 zip codes, the organization is accredited by The Joint Commission, Primary Care Medical Home certified, repeatedly nationally recognized for both its health outcomes and cost efficiency, and provides care regardless of the patient’s ability to pay.
